Library Cards

You are responsible for the proper care and safe return of all items borrowed on your library card and all fines and fees incurred. This includes notifying the library of any change of name, address, e-mail address, telephone number, or other personal data.

How to Get a Card?

For your convenience, there are two ways to apply for a library card. You may choose to do so in person or online: 

  • In Person
    Visit the Grundy Library; you will need:
    • A completed library card application form
      Individual Application Form
      Group Application Form
    • A valid photo identification card
    • Proof of your address if not shown on your i.d. (examples include a copy of a bill or lease)
    • Individuals under 14 years of age will need a parent/guardian present when signing-up for a library card
  • Online
    Complete an Online Library Card Application

Lost Your Card?

To prevent and limit misuse, library card holders are urged to notify the library as soon as possible when a card is lost or stolen. The library will immediately suspend the card when notification is received. Card holders are responsible for all material checked out on a lost or stolen card up to the time when the library is notified.

There is a $3 replacement fee for lost cards.
